How to Set Up the Docnote Custom Form in PS Suite EMR

Learn how to configure the Docnote PS Suite custom form to manage uninsured service requests directly within your PS Suite EMR.

The Docnote PS Suite custom form allows your clinic to manage uninsured service requests directly within the PS Suite EMR. With it, you can:

  • Send sick notes and patient letters directly to patients
  • Initiate prescription refill requests
  • Submit any other uninsured service requests

Step 1 — Add Your Medical Practitioner Number (MPN) & Signature

⚠️ Note: This step must be completed individually by every healthcare professional who will be signing off on sick notes or patient letters. If your clinic has more than one provider, each provider must log in to their own Docnote account and complete the steps below.

  1. Select the profile icon in the top-right corner of your Docnote account.
  2. A side menu will open. Scroll down to locate the following fields:
    • Medical Practitioner Number (MPN) — this will be your CPSO, CNO, or other applicable licensing number
    • Signature Upload
  3. Enter your MPN exactly as it appears in your PS Suite EMR.
  4. Upload or drag and drop your signature image into the box provided (PNG format required).
  5. Select Save at the bottom of the user profile screen.
  6. A success message will confirm your information has been updated.

Step 2 — Set Up Your Letterhead

💡 Note: If your letterhead has already been configured during onboarding, you can skip this step.

  1. From the left-hand menu, select Office Info.
  2. Select the Edit button in the top-right corner of the Office Info page.
  3. Scroll down to the Letterhead Configuration section and fill in your preferred details.
  4. Select Save at the top of the page to apply your changes.

Step 3 — Import the Custom Form into PS Suite EMR

Part A — Download the Form from Docnote

If you are reading these instructions from the PSS Configuration page in Docnote, simply select the orange "Download .cfm" button at the top of the page.

Alternatively, you can navigate here manually:

  1. Select Office Info from the left-hand menu.
  2. Select the Integration tab.
  3. Select PSS Configuration.
  4. Select Download .cfm — a .cfm file will be saved to your computer.

Part B — Import the Form into PS Suite EMR

  1. Open PS Suite EMR and log in.
  2. Select the Records window.
  3. Navigate to Settings > Edit Custom Forms.
  4. You can either drag and drop the downloaded .cfm file directly into this window (then skip to step 8), or continue with steps 5–7 to import manually.
  5. In the forms menu, select File > Import Form(s).
  6. Select the .cfm file downloaded from Docnote: DnForms-[office name][characters].
  7. Select Choose.
  8. A confirmation message will indicate the form was successfully imported.

⚠️ Important: Do not rename this form. The filename is used to establish the connection between the custom form and the Docnote Toolbar. Renaming it will break that connection.
