How to Add, Edit, or Remove Family Members on Your Docnote Annual Plan

Learn how to add family members during Annual Plan sign-up and how to edit or remove them at any time from your Annual Plan page.

If you would like to include family members on your Annual Plan, you can add them during the sign-up process. Family members can be added when subscribing to a Couple, Senior Couple, or Family plan.

Steps to add family members:

  1. Begin the Annual Plan sign-up process — see How to Sign Up for the Docnote Annual Plan for steps on getting started.
  2. When prompted, select Couple, Senior Couple, or Family as your plan type.
  3. During the sign-up process you will be asked to enter each family member's:
    • First and last name
    • Date of birth
  4. Select the orange "+ Add Family Member" button to add each member.
  5. Continue with the remaining steps to complete your subscription.
💡 Note: As the head subscriber, you do not need to enter your own name and date of birth during this step — only those of the family members you are adding.
⚠️ Important: Eligibility rules for family members vary by primary care provider. If you are unsure whether a family member qualifies, contact a Docnote support agent before signing up.

How to edit or remove family members:

If you need to update a family member's information or remove them from your plan after signing up, you can do this at any time from your Annual Plan page.

  1. Log in to your Docnote account and navigate to the Annual Plan tab:
    • Phone or tablet: tap in the top left → select Annual Plan
    • Computer: select Annual Plan from the left-hand menu
  2. On the right side of the page, under "You are currently enrolled in the following Annual Plan", select the orange "Edit" button next to the family member you wish to update.
  3. From here you can:
    • Update the family member's information
    • Delete the family member from your plan
