Family physician in Sarnia, Ontario, in practice since 2010. Lead of the Sarnia Physicians Family Health Organization.
Reclaiming capacity from the system we already have
Ontario needs new capacity, and new capacity is slow and expensive. John's question for the room: where can each of us reclaim capacity from the system we already have, starting Monday morning?
- 01
The work around the appointment
His practice audit found the clinical work was efficient: EMR, e-prescribing, an AI scribe. The struggle was everything around the visit: phone tag, forms, medication-access requests and disability paperwork, still run on phones, paper and faxes.
- 02
It's an attachment problem
A new patient with an A1C of 9.0 comes with a clinical playbook. The administrative requests that come with that patient don't. Teams hesitate to take on more patients because of the work around the medicine, not the medicine.
- 03
Standardize the workflow, not the medicine
Clinical judgment should stay individual. Intake, routing, communication, tracking and delivery don't need to be done fifty different ways. Build that capacity before raising attachment targets.
The work outside the visit shapes how many patients teams take on
Before the Summit, we asked primary-care teams about the work that happens outside the booked visit. John shared their answers in the talk.
-
84%
say work outside the visit affects how many patients they can take on
-
67%
say it takes a great deal or a lot of their capacity
-
27%
have a standardized process for most requests
-
48%
say working in or alongside the EMR would help them adopt
