# How to Create a Docnote Account

## Learn how to sign up for Docnote in a few simple steps. You'll need your health card number, a valid email, and a phone number to get started.

Creating a Docnote account takes just a few minutes. Before you begin, make sure you are 18 years of age or older and have your health card number handy (version code not required).

**To create your account:**

1. Go to[**https://app.docnote.com/login**](https://app.docnote.com/login) or open the invite link sent by your primary care provider.
2. Enter your **full name**, **email address**, **date of birth**, **health card number** (version code not required), and **phone number**.
3. Create a **secure password**.
4. Read and accept the **Terms and Conditions**.
5. Tap **"Create Account"** — a verification code will be sent to your mobile number or email address.
6. Enter the **verification code** to confirm your account.
7. Your account is now active and ready to use.

**A note on phone numbers:** A mobile number is strongly recommended. If you provide a landline, verification codes and messages from your primary care provider can only be delivered to your email instead.

⚠️ **Important:** You must be 18 or older to create a Docnote account. Children under 18 cannot register.
